In 1999 the Big Shot Challenge/Défi-Échec À La Grippe partnership was formed between the former Region of Ottawa-Carleton Health Department, Homes for the Aged in Ottawa-Carleton, Ontario Long-Term Care Association (Region 2), Ontario Residential Care Association (Region 2) and the Ottawa-Carleton Community Care Access Centre. It was initiated to help combat the devastating effect influenza can have on our community’s long-term health and residential care and health care agencies by improving resident and staff influenza immunization rates. The partnership has been instrumental in implementing three very successful campaigns involving a total of 114 health care organizations in 1999, 165 in 2000 and 152 in 2001.
The Big Shot Challenge/Défi-Échec À La Grippe Campaign goals are to immunize at least 90% of residents in all health and residential care facilities. In addition the program aims to immunize at least 70% of full/part-time employees/patients, contract staff, students doing placement work, volunteers, health professionals (such as physicians) and emergency service workers coming into acute care hospitals, homes for the aged, nursing homes, chronic care facilities/units and retirement residences where care/assistance in the activities of daily living is provided.
Among the 88 facilities legally designated as homes for the aged, nursing homes or retirement residences, 92% of residents (93% last year and 91% the year previously) and 79% of staff (86% last year and 77% the year previously) were immunized against influenza last fall. Seven acute care hospitals participated in the challenge for the second year with 47% of their 16,225 staff being immunized (51% of 15,000 last year). Approximately the same number of staff was immunized both years. Please see Annexes A and B for immunization rates by type of facility/organization.
Big Shot Challenge/Défi-Échec À La Grippe plaques are being awarded to 1 acute care hospital, 7 homes for the aged, (including the four operated by City of Ottawa), 9 nursing homes, 39 retirement residences, 14 health care (staffing) agencies, 2 emergency service agencies and 2 community health centres, health-related associations, etc. Of the 74 award winners, 20 are first time award recipients (Annex C) and 54 are repeat award recipients (Annex D). Honorable mention for immunizing 60-69% of staff and 80-89% of residents is made for another 15 participants: one acute care hospital, three nursing homes, seven retirement residences, three health care agencies and one association (Annex E).
The 2001-2002 influenza season has been moderate and as of March 4th, there had been 13 confirmed influenza A (probably Panama strain) outbreaks, two in acute care hospitals and 11 in long-term care facilities.
If there is an influenza outbreak in a health or residential care facility, only persons who are immunized or taking anti-viral medication are allowed to work in the facility (or affected part of very large facilities).
In the unlikely event that persons insist on working when they should not, City of Ottawa Public Health staff meet with them and if necessary use the Ontario Health Protection and Promotion Act to order them not to work in any health or residential care facility in the city with an influenza outbreak. The Ministry of Health and Long-Term Care, the Ontario Hospital Association and the Ontario Medical Association support the use of such orders.
DISCUSSION
For the third year in a row, the Big Shot Challenge/Défi-Échec À La Grippe has been a real success due to the commitment and creativity of health and residential-care facility management, attending physicians, residents and staff in all health care organizations. City of Ottawa Emergency Medical Services (EMS) is particularly to be commended for achieving 100% staff coverage this year.
This is a good example of the dedication and professionalism of each paramedic in the City of Ottawa to comply with the Ontario Ambulance Act, which requires paramedics to receive an influenza vaccination in the autumn of each year. According to the Chief Medical Officer of Health for the Province of Ontario, Dr. Colin D’Cunha, “the single most effective measure to protect both patients and EMS personnel from the transmission of influenza infection is the annual influenza vaccination. Since ambulance personnel may be exposed to persons infected with the influenza virus, unimmunized EMS personnel may develop influenza and put patients who are under their care at risk for influenza infection”.
This campaign continues to be very successful in improving senior’s health and mobility while decreasing deaths. The campaign is based on the solid evidence of the correlation between high influenza immunization rates and fewer flu outbreaks and deaths. A randomized study in England has confirmed the value of staff influenza immunization. Immunizing 61% of staff reduced the influenza death rate in residents by 41%. Evidence from cohort and randomized controlled trials suggest that each dose of vaccine yields a 1-2 hour decrease in lost work time, an average cost saving of approximately $40 per immunized employee.
This past fall, most health and residential care facilities in the City of Ottawa maintained high levels of average resident/patient and staff immunization rates. Further increases are expected next season in order to reduce influenza-related illness and death among facility residents/patients. Given three years’ success with the Big Shot Challenge/Défi-Échec À La Grippe, it will be repeated in the fall of 2002.
The Big Shot Challenge/Défi-Échec À La Grippe continues to be a successful partnership model with a strong health promotion focus. Key strategies included resource development/ distribution, media promotion, advocacy and individual consultation. Aventis Pasteur Limited, the company supplying the vaccine again this year, provided corporate sponsorship for the translation of several flyers into 14 languages. The new influenza display and banner –“Be a Warrior Against the Flu Bug-Combattez le virus de la grippe!” was used extensively for the promotion of the Big Shot Challenge/Défi-Échec À La Grippe and for the Universal Influenza Immunization Campaign.
FINANCIAL IMPLICATIONS
The vaccine was supplied by the Ontario Ministry of Health and Long-Term Care. Individual facilities and physicians provided additional supplies and arranged for the actual immunizations. Funds for this program are in the Public Health Branch Operating Budget.
